Required Documents
1. HHA Certification of 40 hours or more OR CNA License from the State of Florida
2. Current CPR Certification (American Heart Association, Red Cross, or Health & Safety Institute). Online CPR certification is not accepted.
3. CEU Certificates:
Assistance with Self Administered Medication - 2 hours or more
HIV/AIDS - 1 hour or more
Domestic Violence - 1 hour or more
Alzheimer/Dementia - 3 hours or more (including 1 hour video from DOEA + 2 hours or more from an approved training site, please see DOEA website for information https://elderaffairs.org/resources/alzheimers-disease-and-related-dementias-training/)
4. Permanent Resident ID or Work Authorization ID (if applicable)
5. Professional Certificate of Insurance with Liability Coverage
6. Social Security Card
8. Current Driver’s License or ID Card
9. Current Car Insurance (if applicable)
10. Physical Exam within 5 months old or less. Must state you are FREE OF COMMUNICABLE DISEASE.
11. COVID Vaccination Card (collected if available, but not required)
12. Level 2 Background Screening (No major criminal history accepted)

Responsibilities
Provide compassionate, one-on-one care to clients in their homes, ensuring their comfort and safety.
Assist with activities of daily living (ADLs) such as bathing, grooming, and meal preparation.
Monitor and document patient health and behavior, adhering to HIPAA regulations.
Maintain a clean and safe living environment, including laundry and light housekeeping tasks.
